Community Health Liaison

Job Description

Wheat Ridge Colorado Pediatric Clinic is searching for a Community Health Liaison (CHL) who will be the link between the health care system, social services, and the community to facilitate access to services to support families in obtaining the healthcare and community resources needed to maintain their health.

This is a grant-funded contracted position approved through Dec 2018. Potential for extending the assignment dependent on extension of grant funding.

Start Date: March - April, 2018, for 8-9 month assignment
Pay rate: $20 - $22 / hour dependent on experience
Hours: 8:00 am to 4:30 pm, Monday - Friday

Essential Functions Include, but not limited to, the following:
* Coordinate logistics to support Medical Healthcare plan goals and interventions to improve medical health outcomes.
* Assess family needs to tailor appropriate community supports to ensure families are informed and connected to community resources.
* Provides advocacy and support as needed to remove barriers (i.e. language, financial, understanding of resources available, etc) which prevent families from receiving community services as needed.
* Maintains highly organized files for each client and enter appropriate data into designated data collection systems.
* Continuously expand knowledge and understanding of community resources, services and programs.
* Work with team members to update and maintain a directory of community resources (e.g. food-banks, housing assistance programs, childcare resources, etc.).
* Maintains reports, provide necessary documents and data by deadlines and upon request.
* Maintain required core competencies, training and professional development standards.
* Protect client confidentiality and privacy as required by HIPAA.
* Other duties and responsibilities may be assigned.

Please note: This is not a social services or mental/behavioral health position. It is a healthcare liaison position connecting clients (children and their families) with community resources to improve and maintain physical health.


EDUCATION: Associate's degree in healthcare, community relations, social services or other related field. Relevant experience, in addition to the required experience, will substitute for the Associates degree on a year-for-year basis.

EXPERIENCE: Two years of relevant experience.
o Bilingual Spanish preferred.
o Demonstrated knowledge of State social service agencies and community resources preferred.
o Prior experience in community health outreach or similar positions within a health care setting is preferred.
o Proficiency in MS Office Suite (Word, Excel, PowerPoint, Outlook)
